How do you make a "generate HTML code" button? (for myspace surveys)?

I want to make a survey for myspace, and I plan on having it on it's own page. Instead of copying the text from the bulletin, I want the user to have to "click here" to take this quiz, then after they fill in their answers, a button saying "Generate HTML" will be there to copy and paste into the bulletin.

Anyone know how to do this?

I have been to sites that offer making free surveys, but I want my own layout and set up. Thanks
