I have 2 questions:

1. What do you call a survey that has something like this:

[x] Your display name is your real name or a nickname.
[] Your display name is something a friend of yours told you.
[] You made your display name.

Then it has something w/ the percentage at the end

2. Is there a website to get these types of surveys?
